  4.                      FLU_SHOT+ Distributor Application Form
  5.  
  6.         This form should be filled out by shareware distribution 
  7.         companies and Computer User Groups,  wishing to distribute 
  8.         FLU_SHOT+.
  9.  
  10.         In order to qualify, you must either be a computer user group or 
  11.         be a legitimate distributor of shareware products.  Computer User 
  12.         Groups should have one of their officers sign this form and 
  13.         enclose some information on your computer user group for our 
  14.         files.
  15.  
  16.         Shareware distribution companies must include a recent catalog of 
  17.         the products currently available through their company or other 
  18.         such literature.
  19.  
  20.         In any case, this letter must be signed and returned, indicating 
  21.         that you agree to the terms of distribution of FLU_SHOT+ outlined 
  22.         herein:
  23.  
  24.         1)  That the FLU_SHOT+ archive will be distributed in whole, and 
  25.         unmodified in anyway.
  26.  
  27.         2)  That the disk your company/user-group sells containing 
  28.         FLU_SHOT+ is not sold for more than $10.
  29.  
  30.         3)  That persons purchasing a disk from your company/user-group 
  31.         will be educated in the shareware concept and will be encouraged 
  32.         to register shareware programs in general.
  33.  
  34.         4)  That our copyright for the entire FLU_SHOT+ distribution will 
  35.         be respected and that the term(s) FLU_SHOT+, FLU_SHOT, and 
  36.         FLU_SHOT++, our trademarks, will be marked as such in all 
  37.         promotional literature, disk labels, and advertising you may do.  
  38.         As such, the appropriate term shall be labeled as "<term> is a 
  39.         trademark of Software Concepts Design" on all such literature, 
  40.         and a copy of such literature will be forwarded to us.
  41.  
  42.         5)  That your company/user-group will not, under any 
  43.         circumstances, distribute any virus, trojan, or virus-like or 
  44.         trojan-like product under any circumstances.
  45.  
  46.         6)   That you will promptly update your inventory when a new 
  47.         release of FLU_SHOT+ is available, and will immediately stop 
  48.         selling all older versions of the program in any form.
  49.  
  50.         If these terms are agreeable to you, please sign below, return a 
  51.         copy of this letter to us at the above address, along with the 
  52.         appropriate information outlined above.
  53.  
  54.         We'll return a signed copy of this form, along with the most 
  55.         recent copy of FLU_SHOT+ once we review your material.
